Can anyone help me?
My engine has had the main bearing cap that sits over the thrust washers machined to accept a third thrust washer.
This is held in place using two small pins and when the main bearing cap is bolted down sits directly over the top of the standard outer thrust washer.
Has anyone come across this before?

Standard fix when the engine has dropped a thrust washer, or as a preventative if an engine is going to be thrashed within an inch of it's life.
